Symptom
After switch boot up, you may see some ports stuck in initializing status and get err-disabled finally.
Reason is sequence timeout.
%ETHPORT-5-IF_DOWN_ERROR_DISABLED: Interface Ethernet1/4 is down (Error disabled. Reason:sequence timeout)
And there wiil be a "ipqosmgr" crash finally.

Conditions
N3k&9K running on a earlier release than 9.3(1)
Further Problem Description
The ports moved to err-disabled state by ethpm duy to ethpm process did not get reply from ipqosmgr
