Symptom
On a C1117 router, the VADSL module may not come online and can get stuck in a "booting" state:

Conditions
This issue can been seen after a reboot of the system, whether that's due to a reload, power-cycle, or upgrade.
Workaround
Rebooting or power-cycling the router generally recovers the VADSL module from the stuck state. Please note that a subslot-specific reload ("hw-module subslot 0/2 reload [force]") does NOT correct this behavior. Users should take caution if remote access is through any interface on the VADSL module as reachability will be impacted if the module is stuck in the "booting" state.
Further Problem Description
The problematic behavior is the result of a timing issue. At a high level, the router tries to configure an NGIO endpoint on the VDSL-side before IOSd sends the L2 MAC address to the Input/Output Module daemon (IOMd).
